Listed below are the specific medical requirements or other needs relevant to my child participating in the excursion. If your child has no medical or other requirements, please write NIL below.

I give my child (above) permission to attend the excursion to the National Zoo & Aquarium Canberra, Lady Denman Drive, Canberra, ACT 2611; on Friday 8th November 2024, with the Koalas and Possums class.


Excursion Details:
Anticipated number of students likely to be attending: 44
Anticipated ratio of educators attending to children attending: 1:8
Anticipated number of staff members attending: 6
Anticipated number of other adults accompanying and supervising children: 4

Date: Friday 8th November 2024
Period the children will be away from the premises: 9:30am - 2:20pm
Departing St Anthony's ELC at 9:30am
Departing The National Zoo & Aquarium Canberra at 2pm, returning to the ELC at approx. 2:20pm.
I give permission for my child to travel by bus with their class teacher/s.

I authorise the teachers in charge to obtain medical assistance, which they deem necessary in the event of an accident should I not be able to be contacted to give approval, and I agree to pay all medical expenses incurred relating to my child.
